The difference matters: cloudy insulin is intermediate- or long-acting, while clear insulin is rapid- or short-acting. NPH has a milky appearance because it contains protamine, a protein that slows absorption. You’ll need to know which type you’re using because cloudy insulin must be gently rolled, not shaken, to mix the suspension before drawing it up. Clear insulin should never be rolled or shaken—just inspect it for particles and use it as is.

Weight-Based Insulin Dosing in Pregnancy

Insulin needs rise as pregnancy progresses, especially in the third trimester when placental hormones increase insulin resistance. A common starting point for gestational diabetes is 0.7 to 1.0 units per kilogram of body weight per day, divided between basal (long-acting) and bolus (mealtime) insulin. For example, a woman who weighs 80 kg might start at 0.8 units/kg/day, or 64 units total: 32 units of NPH at bedtime and 8 units of rapid-acting insulin before each meal.

Your care team will adjust based on your blood glucose log. Most U.S. obstetricians and endocrinologists ask for fasting and post-meal readings (usually one hour after the first bite) and review them weekly. If fasting glucose is consistently above 95 mg/dL or one-hour post-meal values exceed 140 mg/dL, they’ll increase the dose. Don’t tweak doses on your own—hypoglycemia can be dangerous for you and the baby, and the patterns aren’t always intuitive.

Why Insulin Needs Change

Weight-based dosing is a starting point, not a fixed prescription. As you gain pregnancy weight and hormone levels shift, your insulin sensitivity drops. Many women need two or three dose increases before delivery. After birth, insulin requirements plummet almost immediately—sometimes to zero—so your team will likely stop or drastically reduce insulin while you’re still in the hospital.

Practical Tips for Using Insulin Safely

Store unopened insulin in the refrigerator; once in use, keep vials at room temperature for up to 28 days (check the manufacturer’s instructions). Never use insulin that looks clumpy, has changed color, or has been frozen. If you’re using cloudy NPH, roll the vial between your palms ten times before each draw—shaking creates bubbles and makes dosing inaccurate.

Rotate injection sites within the same area (abdomen, thighs, upper arms) to prevent lipohypertrophy, which can make absorption unpredictable. Use a new pen needle or syringe for every injection. And always carry a fast source of sugar, like glucose tablets or juice, in case of a low. In the U.S., your insurance may cover a continuous glucose monitor (CGM) for gestational diabetes, which can help you spot trends without extra fingersticks—ask your provider.
